Dermatology	ERS	No	Take a photo and send it via Consultant Connect - can get responses within 1 hour but generally within a couple of days
District nurse	Email	Yes - search ‘district nurse	If you want them to do bloods, request offline bloods and save the form and then attach to the email. There’s a separate form for additional requests for patients on their caseload. If it needs to be done urgently you can call them and discuss before 4pm.
DVT clinic	Email	No	Call the DVT nurse at KCH to discuss the case and book a same day or next time appointment, then send an email with the details - use ‘create a document’ and basic referral template. Call 07623 901 822 and quote call sign ‘DVT1’ - they will call back to discuss.
ECG/24hr ECG/24hr BP	Appointments	Yes	Now booked on EMIS appointment book at Palace Road Surgery - check the correct ECG form has been saved to notes.
Echo	Email	Yes	The referral form has clear guidelines. If you’re thinking of heart failure, do a pro-BNP and refer directly to the heart failure team on ERS.
Falls	ERS - Geriatric medicine > falls	Yes - ‘adult therapy referral form’ but good practice to also write a letter
Hearing/Audiology	ERS - Diagnostics	Yes - ‘Adult Hearing'	Use the ’search primary care’ button on ERS to find local services. We have a Hearbase clinic in Paxton that runs on Mondays but you can also refer to anyway in Lambeth/Southwark including Specsavers.
Hospital @ Home	Email + phone call	No	Call 020 3049 5751 (number on their page) as early as possible because their availability tends to run out.
Mental Health - Talking Therapies	Email	Yes - ’Talking Therapies Referral Form'	Generally, patients refer themselves via a phone number or online form but you can send a form if you think it would add valuable info. See the ‘Mental Health’ page for more resources.
Mental Health - Lambeth	Email	Yes - ’The Lambeth Single Point of Access (SPA) Referral Form'	Depends on the borough the patient lives in, in Lambeth all referrals are sent to the Living Well Network Hub. This is for issues such as possible psychosis, severe depression, personality disorders and ADHD/autism assessments (although they refer on for these and it can take a year or more). For urgent cases you can call and discuss the case with them. For less complicated cases, such as patients wanting a personality disorder diagnosis, they can actually refer themselves here via the website.
Mental Health - Southwark	Email	Yes - ’Assessment and Liaison Team SLaM'	Again, dependent on borough.
Mental Health - Older Adults	Email	Yes - ‘Community Mental Health Team for Older Adults'	Used for not only general mental health issues in geriatric patients but for the memory service as well.
Ophthalmology	ERS	No	Most ophthalmology referrals will go via the ’triage’ service on ERS unless an optician has advised a referral to a specific clinic. There are also alternative referral pathways for routine and urgent care, such as the Minor Eye Conditions Scheme and the Video Triage Service at KCH - see the Ophthalmology page for details.
Physio/musculoskeletal	ERS - Physiotherapy > Musculoskeletal ’Search primary care'	Yes - ‘Integrated MSK Referral Form'	Make sure you book an appointment and click ’submit’ - these appointments are just for triaging rather than for patient to attend.
Physio - Older Adults/Falls	Email	Yes ‘adult therapy referral form'
Rapid Access Chest Pain clinic	ERS - Cardiology > Rapid Access Chest Pain	Yes - ‘Rapid Access Chest Pain'	Make sure you select the priority as ‘urgent’ not ‘routine’ otherwise the KCH clinic will not be an option.
Smoking Cessation	Email	Yes - ‘Integrated Respiratory Team Referral Form'	Only certain population groups qualify for NHS services, such as pregnant women, parent with a child, manual worker etc. The form ‘GP Stop Smoking Pathway’ on DXS has info on who qualifies.
Spirometry/Community Lung Function Tests	ERS	Yes - ‘Integrated Respiratory Team Referral Form'	Details on how to find the ERS service are on the form. In the pandemic there are no spirometry clinics running.
TIA	Email	Yes - TIA Referral form'	In work hours you can call the TIA nurse - see the form for details.
Occupational Therapy	Email	Yes - ‘adult therapy referral form'
Ultrasound	Email (KCH)	Yes via EMIS- 'X-ray Form' is a template - click document>create letter then select 'X-ray form	Email the form to ultrasound department at KCH There is a walk-in service for adult testicular ultrasound that runs Monday to Friday 8.45am to 9.45am - same place as normal ultrasound.
X-Rays	Email (KCH) ERS (GSTT)	Yes via EMIS- 'X-ray Form' is a template - click document>create letter then select 'X-ray Form'	ForEmail the form to X-Ray department at KCH FRACTURES and PAEDS it is still a walk in service, for everything else including chest X-Rays you need to email the form to the team and they will arrange an appointment.
